The PharmAssist Program

Your PharmAssist Pharmacist Understands Your Concerns

CREATE A PERSONAL HEALTH RECORD

Although physicians and pharmacists keep thorough records of their patients health, individuals should also keep this information on hand. Maintain a current record of your medications, dosages, health conditions and special requirements. Include your physicians and pharmacist on an "Important Phone Numbers" list. Ensure that a family member or friend is aware of your needs and contacts in case you should ever need assistance.

YOUR HEALTH DOCUMENTS ARE IMPORTANT

Keep your insurance claim records on file. Remember to always carry your health insurance and provincial health cards on your person. In case of loss, add the contact phone and card numbers for health-related cards to your existing credit card list.

TAKE STOCK OF YOUR FIRST AID KIT

Frequently used over-the-counter medications and first aid supplies are often purchased only when they run out. From time to time, review expiry dates and on- hand quantities of first aid items to ensure you're prepared.
